3.  Get to Google Application Services
4. Spreadsheet : Query, Vlookup, Web Published, Hide and Unhide Tab etc
5. Google Form: Create form, Add Question : short, long paragraph, radio button, list, File Upload etc.
6. Use Add-Onn: Form Ranger
